Touchstone Health, a full-service Home Health and Hospice agency, is seeking a qualified and compassionate Speech Therapist to join our Team. Under the supervision of the Administrator and in accordance with the interdisciplinary plan of care, our therapists evaluate, establish, and implement individual programs of care for our patients to increase functional ability and reestablish independence at home.

Duties & Responsibilities:

• To provide services that improve or maintain function in support of the rehabilitation goals defined by the patient’s plan of care in accordance with physicians' orders and therapist assessments.
• Instructs interdisciplinary team members concerning patient’s speech therapy program.
• Maintains records and reports in accordance with the company policies.
• Assists physician in determining patient’s specific abilities and disabilities.
