Hospice Social Worker

Expert Healthcare is seeking a compassionate and skilled Hospice Social Worker to join our dedicated team. In this fulfilling role, you will provide emotional support, counseling, and resources to patients and their families during one of life's most challenging times. Your contributions will be pivotal in helping families navigate the end-of-life journey, ensuring they receive the care and support they need. As a Hospice Social Worker at Expert Healthcare, you will work closely with an interdisciplinary team to assess and address the psychosocial needs of patients and their loved ones. Schedule: Monday - Friday, 40 hours per week.

Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Provide counseling and emotional support to patients and families facing end-of-life issues.
• Connect patients and families with community resources and support services as needed.
• Educate families about the grieving process and provide anticipatory guidance.
• Document all interactions and care provided in accordance with agency policies and regulatory requirements.

Requirements

• Master's degree in Social Work from an accredited program preferred, Bachelor's degree required.
• Current and valid state licensure as a social worker.
• Experience in hospice, palliative care, or related fields preferred.
• Strong assessment and interpersonal skills.
• Ability to work collaboratively within an interdisciplinary team.
• Excellent communication and counseling abilities.
• Compassionate and empathetic approach to care.
• Knowledge of community resources and support services.
