Position Summary
The CALAIM Supervisor | Enhanced Case Manager will provide supervision and support to the CALAIM team while also serving as an Enhanced Case Manager. The CALAIM Supervisor is responsible for ensuring the CALAIM program is implemented effectively and efficiently. As an Enhanced Case Manager, you will work directly with individuals and families, providing case management services to support their access to CALAIM benefits and resources.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Provide supervision, training, and guidance to the CALAIM team members
- Monitor the progress of the CALAIM program and ensure compliance with regulatory requirements
- Conduct case reviews and provide feedback to the team
- Serve as the main point of contact for escalated client cases and complex issues
- Collaborate with community partners and agencies to coordinate services and resources for clients
- Assist clients in completing applications and navigating the CALAIM eligibility process
- Conduct comprehensive assessments to determine clients' needs and develop appropriate care plans
- Provide ongoing case management support, including crisis intervention and advocacy
- Maintain accurate and up-to-date documentation of client interactions and service provision
- Participate in team meetings, trainings, and professional development activities
Requirements
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Bachelor's degree in social work, psychology, or related field
- Minimum of two years of experience in case management or a related field
- Supervision or leadership experience preferred
- Knowledge of CALAIM program rules and regulations is a plus
- Strong organizational and time-management skills
- Exceptional interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficiency in using case management software and computer applications
- Ability to maintain confidentiality and handle sensitive information with discretion
- Cultural competence and sensitivity to the needs of diverse populations
Benefits
The salary for this position ranges from $62,400 to $72,720, depending on experience.
