Case Manager Full Time

Summary

Provide quality case management as part of interdisciplinary treatment team. Serve as a communication liaison between family and treatment team. Develop and implement discharge plans, ensuring housing, transportation, after care arrangements. Participate in treatment planning meetings, communicating patient and family information, placement and aftercare details. Organized and legible documentation.

Requirements

Minimum Education

Bachelors Degree in Psychology or related field.

Minimum Work Experience

Minimum of 1 year behavioral health experience, 3 years preferred. Legible handwriting. Organized and able to handle busy environment.
