Full Job Description
APCares is looking for a full-time Case Manager Supervisor within the Long Beach area. The Case Manager Supervisor will be supervising and monitoring the work of case managers and care coordinators ensuring staff performance towards objectives. This person is required to exhibit advanced skills in training, motivating, communicating with, and assessing employees to supervise and mentor them. The Case Manager Supervisor needs to work collaboratively with other departments to facilitate effective methods of problem-solving and the alignment of strategies.
This is a working position, so the case manager supervisor will work directly with clients and have their own caseload.
Essential Duties and Responsibilities:
The essential functions include, but are not limited to the following:
- Supervise the day-to-day work of case managers.
- Organize performance procedures, standards, and expectations. Reviews them regularly to identify and communicate improvements.
- Prepare and manage extensive assessments for all new participants.
- Ensures the quality and efficiency of casework services by developing effective systems.
- Ensures the team properly identifies clients’ needs, delivers suitable referrals, and ensures the case manager follows up with resources to recommend the program participant’s achievement of the service plan objectives.
- In charge of establishing collaboration and communication with community service providers.
- Execute relevant tasks as assigned.
- Responsible for the management of case files within the Unite Us platform and ensuring case managers keep Unite Us updated with relevant case notes.
Requirements
Minimum Qualifications (Knowledge, Skills, and Abilities):
- Bachelor’s degree in psychology, counseling, human services, or closely related fields.
- A minimum of two years of relevant experience OR
- Must be available for full-time work.
- Solid direct practice, assessment, supervision, counseling, community outreach, team supervision, and project development experience.
- Professional and personal experience with mindfulness or meditation work preferred
- Must express sensitivity to our service communities’ cultural and socioeconomic needs and qualities.
- Knowledge of theories and practice of social work and family assessments
- Technical proficiency with Unite Us platform (Training Provided)
