Join Our Team as a Community Outreach Coordinator!
Are you an experienced Community Outreach Coordinator with a knack for sales? We are excited to welcome you to our dynamic and growing team! In this role, you'll spend most of your day off-site, forging strong relationships with community-based organizations (CBOs), referring physicians, medical offices, and other key community stakeholders in your assigned area.

Key Responsibilities
- Territory Management: Effectively manage your assigned territory, ensuring optimal coverage and engagement.
- Education and Training: Educate CBOs and referring provider offices on new food offerings, enhancements, enrollments, and outcomes.
- Issue Resolution: Identify and resolve service issues related to CBOs, referring physicians, their staff, and patients.
- Network Growth: Grow the CBO and provider network through new provider recruitment and business development opportunities.
- Presentation Preparation: Gather and prepare presentation materials for training sessions.
- In-Service Organization: Organize and conduct office managers' in-service, including training, marketing, and education materials.
- Activity Logging: Maintain detailed visit and activity logs in HubSpot, tracking issues raised during visits.
- Escalation: Escalate unresolved issues to the manager/director for tracking and resolution.
Requirements
- 2-4 years of health plan or management service organization experience in CBO or provider relations
- Bilingual English/Spanish
- Excellent oral and written communication skills
- PC literate, including Google Sheets, Google Docs, Canva, etc.
- Strong customer service skills and experience
- Strong organizational skills
- Excellent interpersonal skills
- Ability to work in a team environment
- Ability to work independently
- This position requires local travel (within 50-mile radius) at least 80% of the time
