POSITION TITLE: Intake Coordinator
REPORTS TO: Director of Account Relationships
LOCATION: Arleta, CA
CLASSIFICATION: Non-Exempt, Full-Time
PERTINENT INFORMATION
- All positions require candidates to successfully pass a background check, LIVE scan and TB test.
- Work Shift Monday - Friday, 9AM-5:30PM.
- Work shift subject to change or multiple shifts may be available in the future.
Intake Coordinator
The Intake Coordinator will manage the referral process serving as a liaison between the LASHA, hospitals, health plan (if applicable), walk-ons or other partnering agency and the NHF Recuperative care staff. The Intake Coordinator will collaborate with LASHA Referral coordinator to ensure a smooth transition from guest initial destination to Recuperative Care facilities. Assures information is gathered from referral partners and/hospitals accurately and thoroughly according to established program policies and procedures. They would be tasked at fostering a positive and collaborative partnership with our partners, providing quality customer service and clear and effective communication.
Responsibilities and Initiatives
To help NHF meet its growth goals, the Intake Coordinator will:
- Serve as point of contact for partners on referrals, providing internal and external updates on referral status. Check and maintain bed reservations, where applicable.
- Ensure accuracy and completion of referral packet received. Retrieve and ensure referrals are entered in a timely manner into a database, confirming partners have received a response within a short time and finalizing referral by letting the facility know that a guest will be arriving.
- Collaborate with LAHSA Referral Coordinator, to ensure timely admission of guests and adherence to the admission criteria.
- Create and initiate guest files in both NHF CRM database and HMIS. Maintain confidentiality of patient information.
- Communicate effectively with team about referrals and admissions.
- Build trusting relationships with partners and guests. Deliver excellent customer service and exercise good judgment with a compassionate, positive attitude while managing a population of guests who may be experiencing difficult life events, such as homelessness, unemployment, addiction recovery, health issues and/or poverty.
- Provide input and evaluation of programming for quality improvement purposes.
- Work collaboratively with other members of the team, including active participation in department and agency staff meetings and training. Assist in providing information and support to the team.
- Project a positive professional image and adhere to organization dress code.
- Flexibility to adapt to schedule changes to meet the needs of the facility.
- Must be able to aid other NHF facilities as assigned by supervisor when the need arises.
- Complete additional tasks as assigned to meet the needs of the facility, notwithstanding primary roles.
Requirements
- High School Diploma required.
- Some knowledge of working with HMIS and LAHSA regulatory requirements preferred.
- 1-3 years’ experience working in a fast-paced, customer service-centric environment.
- Proficient in Microsoft Operating system and database applications; knowledge of CRM software, social media, and website content management systems a plus, but not required.
- Ability to multi-task.
- Detail-oriented with exceptional time management and organizational, written, verbal, interpersonal and computer skills.
- Ability to learn quickly and work effectively with minimal supervision.
- Reliable transportation.
- Bi-lingual in Spanish Preferred.
- TB test required.
- Fingerprint live scan /background required.
Physical Requirements
Work is performed in an office environment and requires the ability to operate standard office equipment such as a computer, keyboard, and telephones. Work is performed for an extended period behind a computer. You will need to be able to talk or hear, in-person, over the phone, or via telecommunication without assistance. On occasion will have multiple lines ringing at the same time. Very limited traveling will occur for the position.
