Full Job Description
Looking for a full-time Licensed Clinical Social Worker (LCSW) for a job opportunity.
We want you to be able to use your skills. Our site positions include roles such as performing and electronically documenting mental health evaluations, progress notes, individual and group counseling notes, and suicide risk assessments, as well as escalating patient needs to providers where appropriate.

Salary: $120k - $130k per year
Responsibilities:
- Provide age and culturally appropriate comprehensive biopsychosocial assessments for mental health, substance abuse, and medical needs in the primary care setting.
- Provide behavioral health services to individuals, couples, and families, including, but not limited to. counseling, intake, assessment/evaluation, diagnosing, treatment planning, crisis intervention and referred to CD treatment.
- Provide liaison services, social work support, and referrals to local resources and community agencies, including medication assistance programs.
- Collaborate with Quality Improvement teams to develop and implement processes for data collection.
Requirements
Qualifications:
- Master’s degree in Social Work and three (3) years post licensure experience
- Must maintain relevant in the state license as Licensed Clinical Social Worker (LCSW)
- Must possess knowledge of counseling techniques, crisis intervention, and could make independent judgment related to counseling as a direct service is essential.
- Experience and skills in group facilitation and planning processes.
