Centrally located in Torrance, California, Sunnyside Nursing and Post-Acute Care provides sub-acute nursing care, rehabilitation services, skilled nursing, and long-term care. Job Description, Includes but not limited to:
- Conduct and coordinate the development and completion of the MDS/RAI assessment in accordance with State and Federal Regulations
- Perform administrative duties such as completing medical forms, reports, evaluations, studies, etc., as necessary
- Evaluate each resident’s condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
- Enter all data into the computer system and transmit all records as required
- Monitor the facility’s QI reports and assist in appropriate action when there are identified actual or potential quality issues.
- Assist the resident and Social Service Director/Designee in completing the care plan portion of the resident’s discharge plan as necessary.
- Conduct or coordinate the interviewing of each resident and or other appropriate resident representatives for the resident’s assessment.
- Develop and participate in the planning, conducting, and scheduling of timely in-service training classes that include assessment skills or techniques needed to complete the assessment functions of the facility.
- Coordinate the development of a written plan of care (preliminary and comprehensive) in accordance with the resident’s needs, State and Federal regulations.
- Assist the Director of Nursing Services and other relevant directors and supervisors in maintaining good rapport and morale between and within departments to Verify that a team effort is achieved in developing and implementing a comprehensive assessment and plan of care.
Requirements
Must have a current and active California LVN License
Must be authorized to work in the United States
Shift:
Monday - Friday
May require some weekends
Prior MDS experience required
