The mission of SOAP MAT, LLC is to provide a high quality outpatient opiate treatment program (OTP) that promotes health and well-being to an underserved opiate dependent population. The objective of the program will be to provide affordable, medically assisted treatment (MAT) utilizing the harm reduction model, focusing on each patient as an individual. The goal of treatment will therefore be to enable the community to function at a higher level, yielding a decrease in crime rates, high risk behaviors and deaths. To that end, SOAP MAT, LLC will promote sobriety, as evidenced by an absence of abusing illicit opioids.
At SOAP MAT LLC, we offer a comprehensive range of services aimed at supporting individuals' overall well-being. Our services include counseling, case management, social work, referrals to community resources, educational programs, work eligibility, vocational rehabilitation, and nutritional counseling. We prioritize compliance with various governmental regulatory agencies, including the Federal Drug Enforcement Administration (DEA), the Substance Abuse and Mental Health Services Administration / Center of Substance Abuse Treatment (SAMHSA/CSAT), the Council on Accreditation (COA), the Food and Drug Administration (FDA), and the California Department of Health Care Services. Job Title: Nurse Practitioner
Reports to: Medical Director, CEO
Supervised by: Clinic Medical Director
Job purpose: The program nurse practitioner is the individual responsible for the medication dosage and regulation of medication for each patient in accordance with all applicable federal and state laws and regulations. He/she is responsible through a supervisory relationship ensuring that all information obtained during the course of physical examinations, laboratory results, and other diagnostic evaluations are recorded in all patient records, to include that all patients’ physical examinations are completed and certified and that each applicant is eligible for treatment. The nurse practitioner reports directly to the clinic director.
- Medication Management: Manage the medical aspects of the clinic services in accordance with local, state and federal guidelines.
- Medical Care: Provide timely, appropriate medical care for patients of the clinic; maintain quality of care; be responsible for all medically related activities within the clinic; assume medical and administrative responsibility for all patients of the clinic, including the signing of patient record notes and treatment plans; induct patients into treatment, initiating, altering and terminating replacement narcotic therapy medications and dosage amounts; determine the appropriateness of dispensing opiate treatment medications for unsupervised (take-home) use; and plan/supervise the provision of treatment, including regular review of and placement of notes in the patients’ records.
- Training / Education: Participate in the orientation and continuing education of the clinic’s staff (including nursing) and perform other teaching related duties as are reasonably necessary and/or appropriate to meet the requirements of SOAP MAT, LLC.
- Regulatory Compliance: Cooperate with the clinic and its designated counterparts regarding the compliance to applicable governmental requirements and in the assurance of the efficient and economical operation of SOAP MAT, LLC. Cooperate with the clinic’s auditors, surveyors, accountants, and financial advisers which is also a critical component of the nurse practitioner duties.
- Corporate Compliance: Participate in, support and comply with all policies, procedures and codes of conduct and sign and adhere to any disclosures or attestations related to the clinic’s compliance program.
- Performance Improvement, Risk Management, and Utilization Review: Assist in the review and evaluation of a data base and statistical analysis in the utilization, efficiency or cost associated with the clinic, and develop, implement and participate in any other quality assurance, performance improvement, risk management or utilization review programs the clinic determines are necessary for the provision of quality services.
- Nondiscrimination: Ensure that all patients presenting at the clinic are appropriately and ethically treated.
- Policies and Procedures: Participate in the development of operational policies, procedures, and protocols for the clinic. The nurse practitioner will also assist in assuring that compliance is met.
- Conflicts of Interest: Report to the company any conflict or potential conflict of interest and provide full disclosure of facts pertaining to any transaction or related activity that may be reasonably construed to be a conflict of interest with the clinic, pursuant to the applicable bylaws, rules, regulations, policies and procedures.
- Audits and Reviews: Participate in and ensure the clinic successfully completes survey, reviews, inspections and audits conducted by accrediting agencies and licensing. This includes governmental agencies, COA, ADP, Drug Medi-Cal / Medi-Cal and DEA.
- Marketing: Assist the clinic director in planning and executing marketing initiatives.
- Cooperation: Perform such other duties for the clinic as reasonably requested by the CEO / clinic director.
- Accountability: In performing the medical services within the clinic, the nurse practitioner shall be directly responsible and accountable to the clinic director for all administrative functions related to medical care. If conflict exists between patient care functions and administrative functions, patient care functions will take precedence. The clinic director shall periodically review the nurse practitioner’s performance as nurse practitioner on an annual basis.
- Absence: It is required that another physician cover for absent nurse practitioner and/or nurse practitioner in the case of illness or vacation.
- Compliance: The nurse practitioner will help SOAP MAT, LLC to comply with all governmental laws, rules and regulations, criteria of COA and applicable bylaws. The nurse practitioner shall not engage in any activity that would cause the clinic to lose its licensure or accreditation.
- Time and Energy: At all times during the term of this agreement, the nurse practitioner shall devote sufficient time and energy to the performance of medical direction services to fulfill those responsibilities in good faith and remain consistent with the terms and conditions of this agreement.
- Records: Timely and accurate medical documentation.
- Confidentiality: The nurse practitioner shall, at all times, preserve the confidentiality of all individually identifiable health care information of SOAP MAT, LLC’s patients, including without limitation, any information which may be obtained or compiled in the operation of any system to identify patients in order to detect and protect against multiple registrations (Protected Health Information). The nurse practitioner will use and disclose Protected Health Information only for the limited purpose necessary for the performance of medical direction services and as permitted by all applicable state and federal privacy protections.
- Reporting: The nurse practitioner shall require that other employees, subcontractors, and agents of the clinic should comply with and recognize all applicable confidentiality and nondisclosure requirements and shall promptly inform the clinic director of any improper use or disclosure of protected health information.
- Medications and Record Keeping: The nurse practitioner shall ensure that the clinic maintains accurate records of medications used in replacement narcotic therapy traceable to specific patients- showing dates, quantity and batch code marks of the medications.
- Retention of Medical Records: The nurse practitioner shall ensure the clinic maintains all records in compliance with federal and state laws.
- Licenses: The nurse practitioner will hold a current medical and DEA license.
Requirements
- Hold a valid and active registered nursing license.
- Possess a master’s degree in nursing, a master’s degree in clinical field related to nursing or a graduate degree in nursing.
- Satisfactorily complete a nurse practitioner program approved by the board.
- Conduct self ethically and professionally at all times.
- Adhere to all state and federal regulations and accreditation agency standards applicable to the individual licensure and narcotic treatment programs.
- Have knowledge of and be culturally sensitive to the problems of the patients served.
- Plan, supervise and regularly review all medical treatment offered in the clinic.
- Ensure that a 24-hour a day emergency contact is available to all program participants (in conjunction with the clinic director).
- Attend treatment team, staff meetings and training sessions on a regular basis as scheduled.
- Admit patients to the program, train staff on medical related issues, sign appropriate documentation.
- Adhere to federal, state and local guidelines.
- Demonstrable history of no alcohol or drug/medication misuse for a period of five (5) years prior to employment. m. The perspective individual will not display any evidence of alcohol or drug/medication misuse and/or unlawful behavior as an employee of SOAP MAT, LLC.

SOAP MAT offers competitive hourly pay from $60-$70/hour.
Monday, Thursday and Friday from 6 a.m.-2 p.m.
