Home Care Assessor/Care Manager (Per-Diem)

Eminence Home Care is looking for a talented Home Care Assessor/Care Manager to join our team!

Eminence Home Care is seeking a dedicated and experienced Home Care Assessor to assist with the screening and onboarding of new clients. The ideal candidate will have a background in care management and possess strong intake and assessment skills. This role involves evaluating clients' functional needs, home environments, and safety, as well as verifying basic funding sources for care. Additionally, the assessor will conduct Quality Assurance (QA) home visits as needed. The assessor will report directly to the Director and will be supported by an office-based team for assignments. Flexibility in scheduling and willingness to travel within the county are essential.

You will:

• Conduct thorough assessments of potential clients to gather information about their physical, emotional, and social needs
• Develop personalized care plans that address the unique needs and preferences of each client
• Collaborate with clients and their families to create a care plan that promotes independence and enhances quality of life
• Coordinate with our caregiving staff to ensure that care plans are implemented effectively and consistently
• Evaluate and reassess care plans on a regular basis to ensure that they are meeting the changing needs of clients
• Document all assessment findings and care plan information accurately and promptly

You will have:

• Bachelor’s degree preferred in social work or related field
• 1-2 years experienced in home care, doing intake/assessing/home visits
• Experience with CT Home Care Program and/or Adult Family Living Program
• Knowledge of DSS Medicaid, or Private Sector Home Care
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
