Full Job Description
Excel Total Business is seeking a compassionate and organized Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and managing the care of clients, ensuring their needs are met and that they receive appropriate support and services. In this role, you will work closely with clients, healthcare professionals, and community resources to develop and implement comprehensive care plans tailored to each client's unique circumstances.
Responsibilities
- Assess clients' needs through interviews, evaluations, and review of medical records and other relevant information.
- Work collaboratively with clients to develop personalized care plans, setting goals and objectives.
- Coordinate and monitor client care and services, ensuring they are delivered effectively and efficiently.
- Collaborate with healthcare professionals, such as doctors, nurses, therapists, and social workers, to ensure continuity and quality of care.
- Advocate for clients and coordinate with community resources to ensure access to necessary support services.
- Monitor clients' progress and adjust care plans as needed, ensuring goal attainment and overall client well-being.
- Maintain accurate and up-to-date documentation, including case notes, progress reports, and care plans.
- Provide education and emotional support to clients and their families, guiding them through the care process and empowering them to make informed decisions.
- Participate in interdisciplinary team meetings and case conferences to share information and collaborate on complex cases.
- Stay current with industry best practices and trends in case management and healthcare.
Requirements
- Bachelor's degree in Social Work, Nursing, Psychology, or a related field.
- Prior experience in case management, preferably in a healthcare or social services setting.
- Knowledge of case management principles, assessment tools, and care planning processes.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage a caseload effectively.
- Excellent interpersonal and communication skills, with the ability to build rapport and establish trust with clients and other stakeholders.
- Empathy and compassion for individuals facing challenging health or social circumstances.
- Knowledge of community resources and services available to support clients' needs.
- Experience working with diverse populations and multicultural sensitivity.
- Proficiency in using electronic medical records and other case management software.
- Ability to work independently and collaboratively in a team environment.
- Professional certifications in case management, such as Certified Case Manager (CCM) or Accredited Case Manager (ACM), are desirable.
