Acentra is looking for a Clinical Care Coordinator to join our growing team.
Job Summary:
The purpose of this position is to provide enhanced care coordination to support care management program delivery to plan enrollees, recommend healthy lifestyle choices and reduce short- and long-term effects of chronic illnesses
Job Responsibilities:
- Ensures the responsible delivery of comprehensive services to enrollees including advocating to resolve issues.
- Participates in the multi-disciplinary team meetings for collaborative assessment and coordination planning to ensure quality care.
- Provides education and resources to beneficiaries/families/providers.
- Proactively engages in delivery of quality management program activities that are the direct responsibility of the Health Services team. Assists in the achievement and ongoing maintenance of accreditations for defined programs.
- Assists Lead, Supervisor and/or Manager in ensuring achievement of contractual obligations, including service delivery in a cost effective and efficient manner and through support of budgetary adherence.
- Report accurate and timely work hours and expenses incurred during course of position duties.
Requirements
Required Qualifications/Experience
- Active, unrestricted current state licensure (RN, LPN) or Master’s of Social Work from an accredited university.
- RN requires minimum 2 years pediatric experience.
- SW requires minimum 1 year of related professional experience.
- LPN requires minimum 4 years of pediatric experience.
Preferred Qualifications/Experience:
- Knowledge and familiarity with public sector health coverage systems, e.g. Medicare/Medicaid.
- Knowledge of principles, ethics and precepts of care management service delivery.
- Strong computer skills, e.g. care management applications, Internet/Web, Microsoft Office (Word, Excel).
- Strong prioritization and organizational skills.
- Ability to demonstrate purposeful and outcomes directed critical thinking within daily operational activities.
- Ability to receive verbal and written feedback in a professional manner and implement performance and productivity improvements as needed.
- Ability to interpret real-time and historical information.

Compensation
The pay range for this position is $30.56
