Job Title: On-Call Inpatient Medical Social Worker
Location: Honolulu, HI, US
Employment Type: Permanent, On-site
Shift: Day shift, 8am - 4:30pm, must work at least 3 shifts per month, weekdays and weekends
Salary: USD $35.00 - $41.00 / hour
Essential Responsibilities:
- Prepares and evaluates assessments, development, and implementation of social work service plans.
- Provides psychosocial assessment, diagnostic impressions, and treatment for individuals/couples/families/groups.
- Performs case management, coordination, casework intervention, and monitoring of social work service plans.
- Assists with administration, development, implementation, and evaluation of social work programs and policies.
- Conducts research and documents according to departmental standards, Federal and State regulations.
- Participates in departmental functions such as staff meetings, staff in-services, and committees, as assigned.
Requirements
Basic Qualifications:
- Education: Masters degree in social work.
- License, Certification, Registration: Licensed Social Worker (Hawaii) OR Licensed Clinical Social Worker (Hawaii), Basic Life Support required at hire from American Heart Association.
- Additional Requirements: Healthcare experience working with medical diagnoses, demonstrated ability in networking with community agencies.
Preferred Qualifications:
- Minimum one (1) year of social work experience in a hospital, medical setting, home health, etc.
- Experience with advanced care directives.
- Experience in insurance referrals.
