What You Will Do
As a Social Worker, you will be responsible for protecting and improving the social and mental well-being and functioning of patients and families by assessing patient needs and connecting patients and families with community-based social services and resources in a multidisciplinary team environment.
The Opportunity
- Participate in IHTC comprehensive clinics to assess the psychosocial needs of patients and families.
- Collaborate with multidisciplinary team members to establish an appropriate plan of care.
- Consult with IHTC providers during patient office visits or emergency situations to provide psychosocial services if needed.
- Educate patients and families about available community resources.
- Act as an advocate coordinating and connecting patients with services and assistance programs.
- Provide therapeutic psychosocial assessment, supportive counseling, crisis intervention, monitoring via telephone, email, texting, virtual and in-person interaction in the home, office, or hospital.
- Contribute to discharge planning process for hospitalized patients.
- Participate in IHTC events, Partners in Bleeding Disorders Education Program, IHTC-sponsored overnight summer camps, and other initiatives as requested.
- Monitor legislation impacting persons with chronic disorders.
- Communicate with outside agencies and organizations to promote IHTC services.
- Coordinate with billing and insurance departments to facilitate resolution of related issues and navigating Medicaid, Medicare, and disability systems, including the application and renewal processes.
This position requires occasional travel to patient homes, schools, and outreach clinics outside of the IHTC. Potential of some travel relating to training or conference events.
Requirements
- Minimum 3 years of related experience in a health care setting required.
- Minimum Master’s degree in Social Work required.
- Valid Licensed Clinical Social Worker (LCSW) in the state of Indiana strongly preferred; valid licensed Social Worker considered if currently pursuing clinical licensure.
- Valid driver's license and automobile insurance is required.
