About Activate Care:
At Activate Care, we’re on a mission to improve health equity and drive improved health outcomes across the country. Our Community Care Record platform enables healthcare and community organizations to coordinate care for populations challenged with health-related social needs. Path Assist is our tech-enabled community health worker program for HRSN utilizing an evidence-based, structured intervention. Our goal is simple: increase health confidence, improve self-efficacy, and reduce inappropriate healthcare spend.
About the Role:
The Director of Operations will manage projects that are considered complex. This is a critical leadership position within our organization focusing on the operations and scalability of our new Path Assist program. A primary responsibility is standing up and managing business units of the Path Assist programs in various locations within the United States. This role collaborates closely with Clinical leadership in the design, execution, management, and ongoing improvement of our clinical care delivery model.
The position will report to the Chief Operating Officer (COO).
Responsibilities:
Process Design & Improvement
- Identify and solve operational gaps in the Path Assist program, services, or systems that ultimately negatively impact compliance, financial, and relationship outcomes. Develop internal processes to support and sustain these improvement efforts
- Work with senior leadership to evaluate service expansion and growth opportunities and execute plans to achieve desired growth
- Partner with clinical, product, and technical leadership to optimize technical tools and integrations in support of care models.
Leadership & Management
- Building new business units in multiple locations in a variety of states simultaneously
- Develop and maintain annual budget and forecasts; monitor financial reports to ensure budgetary compliance with Corporate goals
- Build and manage a scalable operations team

Data Analysis & Planning
- Create systems and dashboards to ensure alignment and sharing of resources, best practices, and efficiencies
- Develop operational standard metrics and KPIs for Path Assist programs
- Design and deliver data and dashboards related to customer and Path Assist program requirements
- Develop an understanding of business-level goals/metrics at the local and national level, focused on maximizing service line performance
- Translation of data into program actions: Ensure programs are data-driven and translate data-driven insights into clear priorities for operational improvements.
- Other miscellaneous duties as assigned
Requirements
Qualifications & Skills:
- Bachelor's degree or equivalent combination of experience and education in organizational leadership, non-profit management, leadership, or related field required
- At least 8+ years of relevant and successful operational management experience
- 4+ years experience in early-stage program design and development
- Proven track record of successfully building new business units
- Strong written and verbal communication skills with the ability to build rapport and effectively communicate with local and remote teams up, down, and across the organization
- Experience using Business Intelligence tools to develop actionable dashboards and reports along with data-driven process improvement plans Creative self-starter with an ability to devise solutions and drive consensus for complex problems
- Experience developing and implementing goals, objectives, and strategic plans
- Versatility, flexibility, and a willingness to work within constantly changing priorities
- Ability to drive results and accountability with local and remote teams; ability to thrive working both independently and within the team setting
- Developing and managing departmental budget
- Strategic planning around operational process design for scale
- Proven ability to develop and maintain collaborative relationships internally and externally with strong interpersonal skills and cross-cultural proficiencies
- Minimal travel required
