RN Supervisor Concierge Nursing Services

• Meet clients and families in their home setting 
• Develop a plan of care including nursing and support services that meet the client's needs
• Determine what care giver team members are appropriate to meet the client's needs and manage that team ongoing
• Conduct comprehensive assessments of clients' health status, including physical, emotional, and psychosocial factors, to ensure holistic care planning.
• Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate seamless delivery of care and optimize client outcomes.
• Monitor and evaluate clients' responses to interventions and adjust care plans accordingly to promote optimal health and well-being.
• Provide education and support to clients and their families on disease management, medication administration, and lifestyle modifications to empower them in self-care and promote long-term health goals.
• Available to client and team on a daily basis to ensure services are provided timely and in full compliance with the clients stated desires.

Requirements

• COVID vaccination required
• Licensed Registered Nurse. BSN preferred
• Experience with wound care, vaccine administration, vents, and tracheostomy
• Supervisory experience
• Prior home care experience REQUIRED
• Private pay experience preferred
• 3+ years clinical experience
• Valid US Drivers License
