Home Health Physical Therapist

Thomas Talent Network, LLC is currently seeking a compassionate and experienced Home Health Physical Therapist to join our team. As a Physical Therapist, you will provide care and treatment to patients in their homes to help them regain or maintain their physical abilities and improve their overall quality of life. You will work closely with patients, families, and a multidisciplinary team to develop personalized treatment plans and provide ongoing support and education.

Responsibilities:

• Conduct comprehensive assessments of patients' physical abilities and limitations
• Develop individualized treatment plans based on patients' goals and needs
• Administer therapeutic exercises and activities to improve patients' strength, range of motion, balance, and coordination
• Utilize therapeutic modalities, such as electrical stimulation and ultrasound, as appropriate
• Educate and train patients and their families on proper techniques, exercises, and equipment use
• Monitor patients' progress and adjust treatment plans as needed
• Document patients' evaluations, treatment plans, progress notes, and other relevant information
• Collaborate with other healthcare professionals to coordinate patient care
• Adhere to all state and federal regulations and agency policies

Requirements

Requirements:

• Valid Physical Therapist license
• Minimum of 1 year of experience as a Physical Therapist, preferably in a home health setting
• Strong clinical and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Valid driver's license and reliable transportation
• Flexibility to travel to patient homes within the assigned territory
