Pay Rate: $25.38 per hour
RADIAS Health is hiring a part-time Case Manager to join the Targeted Case Management team!
The Case Manager provides person centered services to promote recovery and community integration of adults with serious persistent mental illnesses and co-occurring substance use disorders within Ramsey County. Our goal is to help them obtain and maintain an optimum quality of life that is consistent with the person's needs and preferences. Case Managers help clients gain access to needed medical, social, educational, vocational and other necessary services as they relate to the client's mental health needs.
Hours: 20 hours per week, Mon - Fri flexible day hours
The Case Manager position is a hybrid model. Some work is done in the community with clients and other work can be done from home.
Duties & Responsibilities
- Develop functional assessments and community support plans
- Talk with clients about their mental health symptoms and service needs
- Link clients with needed psychiatric and medical providers
- Refer clients to community resources
- Assist with financial and healthcare benefits, and communicate with providers and family members
- Monitoring civil commitments, writing required reports, and communicating with Ramsey County court programs.
Requirements
- Bachelor's degree in behavioral sciences or related field (if no degree, 30 semester credit hours in behavioral health sciences) and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders - OR-
- Masters or other graduate degree in behavioral health sciences or related field -OR-
- If no degree, must have 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders
