RADIAS Health is seeking a passionate and motivated Program Director to lead our Whatever It Takes (WIT) and Community Competency Restoration (CCR) teams! Whatever IT Takes (WIT) specializes in assisting individuals with serious and persistent mental illness discharged from state institutional settings and back into the community and the Community Competency Restoration (CCR) program assists clients in the criminal justice system to obtain and maintain an optimum quality of life consistent with client needs and strengths. As the Program Director, you will play a crucial role in overseeing and managing both programs.
The Program Director will provide both direct client service time and be responsible for supervising staff. In this role, you will provide clinical, recovery-based transition services to expedite discharge from state facilities, local hospitals, and IRTS for individuals deemed clinically ready for discharge, develop individual community reintegration plans, and ensure successful community living for clients. The Program Director will supervise a team of multidisciplinary practitioners and partner with counties, tribes and other community providers to leverage existing resources and/or develop new services for our clients.
Hours: Monday - Friday 8 am - 4 pm or 9 am - 5 pm
This position works hybrid - some work is done in the community with clients and other work can be done from home.
Duties and Responsibilities:
- Responsible for all clinical, organizational, and financial aspects of the programs.
- Develop case conceptualizations based on record reviews, client interviews and consultation with providers in order to develop community reintegration plans that leverage strengths, mitigate barriers, and are based on individuals' needs and choices.
- Coordinate clinical transition plans with the county, state, and involved providers and procure the identified resources necessary for discharge and successful living in the community.
- Act as a liaison to counties and the MN Department of Human Services with all clinical, contractual, and funding matters.
- Provide clinical direction to team members for the day-to-day clinical operations.
- Coordinate program activities with community providers and other agencies/entities to ensure optimum level of communication and care of the client.
- Establish and maintain professional relationships with the State and County providers to ensure an accurate understanding of the WIT grant and appropriate use of services/funding in client discharge planning.
- Establish and maintain professional relationships with Ramsey County leadership to ensure accurate understanding of the contract deliverables.
- Provide psycho-education to community providers/family to help ensure that they can effectively support each client on an ongoing basis in the community.
- Ensure standards are met according to State and County guidelines. Ensure appropriate reporting is submitted to Vulnerable Adult, Child Protection, the Ombudsman’s Office, or other agencies/entities when necessary.
- Meet regularly with State and County administration on program development and coordination.
- Develop and administer programs budget including line items, and to cover travel and transportation costs.
- Develop and maintain program policies and procedures and revise as necessary.
- Ensure that required program evaluations are completed.
- Develop and ensure completion and compliance of all mandated training. Identify and/or conduct any necessary individual training for direct reports.
- Responsible for keeping on-going performance documentation for all direct reports.
- Hiring of all positions in concert with Human Resources Manager and Recruiter.
- Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
- Participate actively in on-going professional growth and development, maintain appropriate professional behavior, and participate in appropriate supervision
- Be able to transport clients in the community.
Requirements
- Master's degree in Behavioral Health Sciences
- Licensed Mental Health Professional in the State of Minnesota (LICSW, LP, LPCC or LMFT license)
- 5 years of experience providing services to individuals with mental illness
- Must have car, auto insurance, and valid drivers license
