The Forensic ACT Manager will provide professional, quality program services to the clients served by the FACT team. Work involves the identification, development, implementation, and evaluation of program policies and procedures as directed by the Program Director, as well as professional practice standards. Work is performed with latitude for independent judgment, and action is reviewed by the supervisor through conferences, reports and observation of results obtained.
Job Duties & Responsibilities:
- Direct the day-to-day clinical operations of the FACT team including scheduling staff work hours to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hours; lead the daily organizational staff meetings and treatment planning meetings; continuously evaluate the status of clients and do appropriate planning and coordination of treatment activities to ensure immediate attention to their changing needs.
- Direct and coordinate the client admission process as well as the treatment, rehabilitation, and support services of the program in coordination with the program director. Schedule the admission interviews in the Corrections facility; develop and coordinate the initial assessments and initial treatment plan in collaboratively with the DOC; assign the most appropriate staff to the individual treatment team; and provide clinical supervision for the development of the comprehensive assessment and the treatment plan for each client.
- Direct, coordinate, and review all client-centered comprehensive assessments, treatment plans, and progress notes. Train staff on the development and maintenance of all documentation and reports.
- Supervise medical records management assuring compliance with agency policies, Medicaid, and other third-party insurance requirements.
- Carry out and document quality-assurance activities and review the use of program services.
- Initiate and maintain relationships with law-enforcement and other human-services agencies and with informal community resources (e.g., landlords, employers).
- Conduct clinical assessments and provide direct clinical treatment, rehabilitation, and support services to clients.
- Participate at least 50% time in direct services.
- Be responsible for meeting reporting outcomes and FACT fidelity standards.
- Perform on-call duty for crisis intervention and for staff supervision and consultation and outreach.
- Must be able to transport clients in the community.
- Share clinical supervision of the team with the Program Director.
- Supervise all staff and complete performance evaluations, growth and development plans, discipline plans, and recommendations for salary adjustments.
- Hire all positions in concert with Human Resources Manager and Recruiter.
- Responsible for keeping on-going performance documentation.
- Develop and ensure compliance of all mandated training. Identify and/or conduct any necessary individual training.
- Maintain a pattern of regular work hours.
- Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
- Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
- Other duties as assigned.
Requirements
- Master’s degree in the behavioral sciences or related field.
- Must be qualified as a Mental Health Professional maintaining a LICSW, LP, LPCC or LMFT license preferred; or hold a current and valid national certification as a Certified Psychiatric Rehabilitation Practitioner and working toward independent licensure to be earned within 24 months of assuming role of Team Leader.
- Must have a minimum of 6,000 hours experience working with adults with serious and persistent mental illness.
