Sign On Bonus! $5000 paid out at 6 months.

Part-time: 16 Hours per week

The Psychiatric Nurse Practitioner will provide professional, quality services to the clients served the PATH team. Diagnose, prescribe medication, monitor medication side effects, and participate in the treatment process of clients served by the PATH team.

Duties & Responsibilities:

  • Provide psychiatric education, support, and care to people experiencing homelessness in locations such as drop-in locations, shelters, encampments, and other places not meant for human habitation where humans are residing.
  • Work within a multidisciplinary team consisting of Mental Health Professional/Practitioners, Certified Peer Specialist, etc.
  • Prescribe short-term psychiatric medication – refills or replacement medications for individuals within our unsheltered community with immediate needs and will work with outreach staff in effecting timely referrals to community resources able to provide ongoing care to clients.
  • Coordinate psychiatric care for clients that are PATH eligible and/or enrolled in PATH services. Prescribe medications and evaluate clients for side effects and medication efficacy.
  • Monitor clients who may decline active services who represent a potential danger to self or others to ensure referral for emergency psychiatric or other crisis intervention services as needed.
  • Work with outreach staff in effecting referrals to community resources, office-based providers and/or specialists for ongoing care.  
  • Participate in case consultation meetings.
  • Develop individualized treatment plans in collaboration with the outreach team to address the specific needs and challenges of each participant. This includes medication management, therapeutic interventions, and psychoeducation.
  • Be actively involved in both the acute and long-term psychiatric inpatient treatment of PATH clients, including initiation and facilitation of admission. Provide regular communication and consultation with the attending psychiatrist. Along with other team members, collaborate with inpatient staff on the planning and implementation of treatment and discharge to the community.
  • Regularly follow up with participants to track their progress, monitor treatment effectiveness, document accurately and adjust interventions as needed.
  • Train staff on the knowledge and skills basic to the treatment of serious and persistent mental illnesses, psychopharmacology, and clinical medical issues.
  • Provides initial care of emergencies and arranges for continuing definitive care
  • Assist in developing and monitoring psychopharmacological and medical treatment.
  • Participate in the development of medication policies and procedures.
  • Complete all paperwork, reports, and charting in a timely and organized manner.
  • Work collaboratively with other agency prescribers as needed to support peer consultation and shared knowledge on practice trends.
  • Conduct Diagnostic Assessments as needed. The Diagnostic Assessment shall include psychiatric history, course of illness, response to treatment, mental status examination, use of DSM 5 diagnoses, a summary of strengths and needs recommendation.
  • Provide crisis assessment and management during regular work hours.
  • Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
  • Maintain a pattern of regular work hours.
  • Other duties as assigned

Requirements

  • Graduation from an accredited Advanced Nurse Practitioner program, with relevant board- certification and an active unrestricted license to practice as an APRN in Minnesota.
  • An active full scope APRN license and furnishing credential within the state of Minnesota is required.
  • Knowledge and experience in providing psychiatric and addiction related care to people from diverse cultural backgrounds.
  • Knowledge and experience in providing street medicine to people experiencing homelessness.
