Limited Duration Timed Contract
To provide quality program services to the persons served by RADIAS Health homeless services. This position is intended to expand the capacity of RADIAS Health Outreach services, partnering with other team members in providing in-person outreach services to individuals/couples in shelter as well as ones experiencing unsheltered homelessness in Ramsey County.
Duties & Responsibilities:
- Provide highly mobile street outreach to those experiencing homelessness; including in shelter, day drop-in's, on transit, at encampments, detox, jail, etc.
- Assess an individual's needs, build rapport and develop appropriate care plans to assist in connecting to stable housing and support one in achieving their individual goals
- Engage in coordinated outreach efforts with RADIAS Health Homeless Outreach Team members and other community service providers.
- Initial assessment to determine presence of mental illness, chemical dependency, or behavioral problems.
- Coordinate service provisions with other service providers to ensure smooth, prompt, consistent and quality delivery of services to individuals.
- Refer urgent cases where there is risk of harm to self or others to the appropriate hospital emergency room and/or Crisis.
- Provide information and referral, intervention strategies and assistance in deescalating the crisis, if there is no imminent risk of harm to self or others.
- Provide Client Advocacy and Information and Referral to clients and agencies to ensure protection of civil rights and welfare of clients.
- Refer cases to Adult Protection and Child Protection where there is evidence of neglect and/or abuse of Vulnerable Adults and/or children.
- Assist clients in obtaining needed legal and/or advocacy services.
- File all legally required reports with Vulnerable Adults, the Ombudsman and Child Protection.
- Provide education to clients regarding the symptoms, course and treatment of their illness.
- Attend required meetings and trainings as scheduled.
- Must be able to transport clients in the community.
- Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
- Maintain a pattern of regular work hours.
- Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
- Demonstrates appropriate professional boundaries with colleagues and clients.
- Allow for significant flexibility in hours worked, occasional evenings as needed
- Other duties as assigned.
Requirements
Required:
- High School Diploma
- Experience supporting persons experiencing homelessness or lived experience
Preferred:
- Bachelor's degree in behavioral sciences or related fields; and
- 2,000 hours of supervised experience in the delivery of services to persons with mental illness.
- Experience in working with people who are chronically homeless
- Knowledge with the Coordinated Entry System
- Experience with HMIS and CES Assessments
