Full Job Description
- We are currently looking for a Full Time Support Coordinator to support individuals with disabilities in Atlantic, Monmouth, Burlington and Middlesex counties. The successful candidate will be responsible for assisting participants to gain access to needed programs, services and supports including but not limited to medical, social, educational and other services. The Support Coordinator in conjunction with the participant will also develop Individual Service Plans and maintain ongoing contact to assure services are being provides to the individual's satisfaction.
- This position is partially remote. Applicants will have an in-home office set up but will be expected to meet clients in the various counties we support for in-person meetings. To be considered, you must live within Ocean, Monmouth, Middlesex, Burlington or Atlantic Counties.
Duties:
- Utilizes various assessments, discovery tools, interviews etc. in order to assist the planning team to identify needed services and supports; schedules and facilitates planning team meetings in collaboration with the individual.
- Completes/writes the Person Centered Planning Tool (PCPT) and Individual Service Plan (ISP) with input from the participant, their family and other team members designated by the participant.
- Notifies individual, planning team and service provider when services are changed, reduced or terminated. Revises PCPT and ISP as necessary.
- Researches, locates options for services that may include day supports, transportation, individual supports, generic community and natural supports, other state or federal funded supports.
- Links participant to service providers by providing information, confirms service capacity, intake and eligibility requirements; establishes provider ability to implement strategies to reach identified ISP outcomes and/or other actions to establish service delivery.
- Provides monitoring and follow up to ensure delivery of quality services, ensuring services are provided safely and in full consideration of individual’s rights and to individual’s satisfaction.
Requirements
- Bachelor’s Degree in Human Services, Special Education or related area required.
- Must posses a New Jersey drivers license with 5 points or less.
- Minimum of 2 years experience working with individuals with disabilities
- All employees are required to be fully vaccinated and up to date with vaccine status
