Xpress Wellness Urgent Care is seeking a highly motivated and experienced Community and Facility Medical Coordinator (LPN or MA) to join our team. As a leading healthcare provider in Oklahoma, Xpress Wellness offers comprehensive urgent care services for non-life-threatening illnesses and injuries. In addition, we provide occupational medicine, workers compensation, behavioral health, virtual primary care, and facility-based primary care.
The Community and Facility Medical Coordinator position will be responsible for a variety of clinical and administrative duties. This will include updating and maintaining patient records, managing provider schedules at nursing facilities and ensuring serial compliance. Facilitating specialty referrals, imaging needs, and medication authorizations as indicated, and communicating effectively with patients and family. This position will work closely with medical providers, outside facility nursing, administration, and admitting staff to ensure quality patient care. Coordination with the internal Xpress Wellness RCM team and operations will be critical. The position will ideally be located in Oklahoma.
Responsibilities:
- Participate with leadership in developing, improving, and growing medical care in long-term facilities.
- Assist in ensuring the accuracy and timeliness of patient data meets company goals.
- Coordinate the implementation of new tools and resources with the medical care in long-term facilities team.
- Communicate issues and concerns with the Director and operations team.
- Work with the RCM team to coordinate the flow of accurate patient information from the clinical team to the billing team.
- Works with long-term care facilities to correct problems, gain efficiencies, and coordinate the flow of needed information.
- Assist with credentialing of all providers by assisting with gathering needed information.
- Respects and acknowledges clients rights and maintains strict confidentiality in all client matters; Investigates and resolves incidents and issues as required while maintaining HIPAA compliance and associated relevant guidelines and regulations.
- Other duties as assigned, this is a safety sensitive position.
Requirements
- High School Diploma, or equivalent, required. Associates or higher preferred.
- Strong communication, organizational, and problem-solving skills.
- Proficiency with Microsoft Office suite and Electronic Medical Records. Will need to be technically savvy and comfortable using tools such as mobile iPads for charting.
- 3+ years of clinical experience in a medical office or nursing facility preferred.
- CPR certification preferred.
- Medical Assistant certification preferred.
