Job Summary
The Community Liaison is responsible for planning and implementing a networking and marketing strategy to the community and professional referral sources to generate qualified referrals. The Liaison is responsible for the accurate dissemination of information regarding the hospice benefit to the community, potential patients and referral sources. This position is responsible for promoting a compassionate and credible image to the community for both themselves and BeLoved Hospice.

This position reports directly to the Director of Marketing & Business Development
Job Duties and Responsibilities
Specific to Position:
- Has a solid understanding of basic hospice regulations, levels of care, and admission criteria.
- Utilize community resources to research and identify referral sources, potential hospice physician relationships, and missed marketing opportunities in the community.
- Has the ability to create new relationships by constantly seeking out new referral relationships and opportunities in the communities served.
- Ensures that all marketing materials are current and up to date with BeLoved Hospice policy and current regulations. Notifies management if any marketing material is out-of-date or incorrect.
- Plans for and achieves monthly goals for new lead sources, referrals, and admissions.
- Proactive, organized, timely and appropriately persistent in pursuing leads and follow-up.
- Gather data on patient referrals to assist with the admission process.
- Ability to make “cold calls” in person and by telephone.
- Develops appropriate call plan to achieve pre-established objectives. Conducts a minimum of forty (40) sales calls and presentations each week and generates referrals by building long term business partnerships.
- Uses appropriate established techniques for sales calls including opening, probing needs, presentation, handling objectives, and closing. Meets admission targets consistently on a monthly and quarterly basis.
- Excellent listening skills and ability to understand and empathize with patients and their families and the professional referral community.
- Works with the Admissions Team to ensure that patient admissions are completed in a timely and accurate manner. This could include attending the admission, obtaining consents, obtaining attending physician certification of terminal illness and other admission duties to be assigned when necessary,
- Coordinates effective community education programs to increase community awareness on the benefits of hospice and palliative care and to help generate qualified patient referrals.
- Direct or assist with the coordination of community awareness events including health fairs, health care mixers, and BeLoved Hospice sponsored community events.
- Acts as a liaison between physicians, referral sources, and other referral partners to report patient’s progress towards goals, current condition, and updates to care plans when appropriate.
- Maintains a positive attitude, energy, enthusiasm, initiative, fortitude, perseverance, and follow-through with leads and referral sources within the community and always projects a positive, compassionate image as a representative of BeLoved Hospice.
- Maintains above the line standards by only representing the positives of BeLoved Hospice and never indulging in or focusing on negative banter about other agencies, their employees or referral sources, even when it might be true.
- Other duties as assigned.
Physical/Safety Requirements:
- Participates in emergency procedures, including disaster drills.
- Operation of equipment in a safe manner and shares responsibility for communicating safety violations or unsafe practices to management.
- Equipment Used: General office equipment (telephone, copy machine, computer, fax machine) and motor vehicles.
- Category 2 for potential exposure to blood/bodily fluids.

- Utilizes appropriate PPE, as needed.

- Must possess sight/hearing/speech senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position are fully met.
- Use appropriate body mechanics as needed for personal use.
- Utilize appropriate basic office equipment.
Requirements
Qualifications and Experience
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required to perform this job.
- Be an experienced sales professional, where relationship-building skills (listening, understanding, helping to identify appropriate solutions and closing) have been proven.
- One-year experience is preferred in hospice or health services, to assure understanding of hospice care and the overall continuum of care for hospice patients.
- Must have reliable transportation and maintain auto insurance.
- Successful completion of background check pursuant to ORS 181A.195.
- Read, write and understand the English language.
- Must be able to handle frequent interruptions, problem solve, and have exceptional follow through skills.
- Must be able to cope with emotionally charged situations encompassing patients, families, caregivers, and team members.
- Must be self-motivated and able to work independently, effectively managing time for maximum results.
