Full Job Description

Access Services is hiring a full time Assessment Coordinator for the Justice Related Services (JRS) program serving individuals in Montgomery County, with an office based out of Plymouth Meeting, PA.

At JRS, we believe recovery in mental health is real, second chances are possible, and successful reintegration into the community is achievable. We practice a person-centered approach of support and encourage adults with serious mental illness to use their own strengths in achieving success and independence. Our caseworkers collaborate to help adults with mental health navigate the forensic system, whether currently incarcerated or navigating re-entry into the local community. We do this by guiding adults out of incarceration with re-entry plans, and we divert incarceration for other adults by connecting them with the mental health treatment they need.

Our Assessment Coordinator will be a critical team player in allowing us to carry out our JRS mission and the Access Services philosophy of care so that our services meet each individual's needs. This teammate will assess and coordinate services for adults with serious mental illness. The Assessment Coordinator will collaborate with the pre-trial unit, adult probation, and the forensic system partners of Montgomery County and connect each individual to the proper treatment and support for their success.

Work Location:

This is a full-time position, and will require a physical presence in the Plymouth Meeting, PA office as well as some travel as needed to provide services across Montgomery County.

Requirements

Essential Functions

  • Engage adults with serious mental illness and involvement in the forensic system in order to assess needs and facilitate connection to needed support.
  • Utilize and complete ASAM assessments, case notes and documentation when appropriate.
  • Act as a liaison, collaborate, and support coordination with pre-trial and forensic partners, and facilitate multisystem meetings and support for complex cases.
  • Build collaboration through community outreach and awareness.
  • Attend county, committee and consumer meetings as needed.
  • Obtain updates for pre-trial and data collection, and maintain updated documentation.
  • Hold open hours in collaboration and attend regular team meetings and case reviews.
  • Complete all outcomes, statistical reports and billing documents as needed.
  • Participate in program audits.
  • Maintain regular and predictable attendance, and work scheduled overtime, or be available for unplanned overtime as necessary, to meet program needs.
  • This position requires the ability to travel during the course of a normal workday to successfully carry out the expectations of this position including attending offsite meetings, providing services in the community, and providing service oversight and supervision. Therefore, employee will need to drive their personal vehicle and/or an agency owned vehicle to meet these expectations.

  • Proficiency in, or ability to learn and use, workplace applications, platforms and technologies.

Requirements and Qualifications

Education: Bachelor’s degree in social work or a related field required, Master’s degree preferred.

Experience: Experience working with adults with serious mental illness, experience in clinical assessment, and experience working with the criminal justice system preferred.

Certification: Must be willing and able to complete ASAM certification* within six months of hire.

*The ASAM criteria (American Society of Addiction Medicine) is the most widely used and comprehensive set of guidelines for placement, continued stay and transfer/discharge of patients with addiction and co-occurring conditions.within six months of hire. This criteria views patients in their entirety, rather than a single medical or psychological condition. When determining service and care recommendations, the ASAM Criteria pays attention to the whole patient, including all of his or her life areas, as well as all risks, needs, strengths, and goals.

  • $21/hourly, with rates negotiated on education and experience in the field.
