Primary Job Responsibilities:
- Provide nursing care in an inpatient/residential hospice facility.
- Determine the scope and frequency of services based on patient acuity and family needs.
- Identify and address patient care needs, focusing on pain and symptom management, education, and compliance.
- Promote maximum quality of life for patients within their limitations.
- Enhance patient and family adherence to the plan of care.
- Coordinate comprehensive patient care through a team approach.
- Establish care plans for CNAs/HHA and supervise them as required by agency policy.
- Stay updated on nursing practices through educational programs and in-services.
- Document care accurately and timely, ensuring all necessary information is recorded.
- Maintain patient confidentiality and uphold patient rights.
Requirements
- Ability to be flexible and empathetic towards others.
- Graduate of an accredited nursing program.
- Current RN license in good standing to practice in Pennsylvania.
- Minimum of one year of experience in homecare or a medical/surgical setting.
- Experience in pain and symptom management.
- Understanding of hospice philosophy and principles of death/dying.
- Proven clinical and professional competence.
- CPR certification required.
- Reliable transportation.
- Ability to follow oral and written instructions effectively.
- Strong communication and organizational skills.
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
