Primary Job Responsibilities:
- Assess patients and develop individualized care plans based on their medical condition and goals of care
- Manage complex symptoms, including emotional, physical, and existential components, to improve patients' quality of life
- Collaborate with physicians, interdisciplinary care teams, and patients' families to ensure coordinated and effective care
- Recommend and implement pharmacological and non-pharmacological interventions
- Monitor patients' response to treatment and make necessary adjustments to the care plans
- Educate patients and families on their diagnoses, prognosis, and treatment options
- Maintain accurate and timely documentation of patient encounters
Requirements
- Graduate of an accredited school of nursing and nurse practitioner program
- Current license in good standing to practice as a Nurse Practitioner in the Commonwealth of Pennsylvania
- Minimum of two years of experience as a registered nurse or nurse practitioner
- ACHPN certification required or the ability to obtain within six months of hire
- Demonstrated clinical and professional competence
- Excellent communication skills and the ability to effectively collaborate with the care team and patients' families
- Strong organizational skills
- Valid driver's license and reliable transportation
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
