Primary Job Responsibilities:

  • Assess patients and develop individualized care plans based on their medical condition and goals of care
  • Manage complex symptoms, including emotional, physical, and existential components, to improve patients' quality of life
  • Collaborate with physicians, interdisciplinary care teams, and patients' families to ensure coordinated and effective care
  • Recommend and implement pharmacological and non-pharmacological interventions
  • Monitor patients' response to treatment and make necessary adjustments to the care plans
  • Educate patients and families on their diagnoses, prognosis, and treatment options
  • Maintain accurate and timely documentation of patient encounters

Requirements

  • Graduate of an accredited school of nursing and nurse practitioner program
  • Current license in good standing to practice as a Nurse Practitioner in the Commonwealth of Pennsylvania
  • Minimum of two years of experience as a registered nurse or nurse practitioner
  • ACHPN certification required or the ability to obtain within six months of hire
  • Demonstrated clinical and professional competence
  • Excellent communication skills and the ability to effectively collaborate with the care team and patients' families
  • Strong organizational skills
  • Valid driver's license and reliable transportation
  • Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
  • Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
