Position Overview

Acentra is seeking a dedicated Pediatric Coach to join our expanding team for the Pediatric Complex Care Resource Centers (PCCRC). The Pediatric Coach plays a pivotal role in providing essential support and intervention to caregivers and families who are caring for children with complex medical needs in a home setting. With a focus on non-medical aspects, the Pediatric Coach tailors and delivers education to caregivers, equipping them with the necessary resources for successful home care.

In addition to providing direct support, the Pediatric Coach collaborates closely with other support entities to address challenges in care delivery proactively. Active participation in meetings and collaboration with stakeholders is essential to ensure comprehensive care coordination.

This role entails regional travel, including potential overnight stays, and may require flexibility for evening and weekend engagements.Candidates must reside in the southwest quadrant of Pennsylvania to be considered for this position.

Job Responsibilities

  • Collaborate closely with the child’s caregivers to comprehensively understand their care concerns and aspirations for the child. Through effective teamwork, facilitate tailored education sessions aimed at crafting the optimal support plan for the child's well-being.
  • Develop, meticulously plan, and deliver person-centered training sessions, employing diverse formats, to families and healthcare providers who champion children with complex medical needs in a familial environment. Ensure thorough documentation of all training activities.
  • Harness department-approved training modules and resources to provide empowering education to caregivers and the broader community, fostering an environment conducive to optimal support for children with complex medical needs.
  • Spearhead the development and orchestration of a regional Family Advisory Group in adherence to contractual obligations. Actively engage in leadership roles within the group to ensure the voices of families are heard and valued.
  • Effectively collect and meticulously maintain data, leveraging it to craft comprehensive reports that provide valuable insights to stakeholders. Through insightful analysis, contribute to the continual enhancement of care delivery practices.
  • Demonstrate commitment to professional growth and collaboration by actively participating in local, regional, and statewide project meetings. Contribute insights and expertise to collective efforts aimed at advancing pediatric care standards and practices.

Required Qualifications, Skills, Knowledge and Experience

  • Bachelor’s degree in Education/Special Education, Social Work or Psychology.
  • Possession of appropriate state licenses as mandated by the Pennsylvania Licensing System for education or profession.
  • Minimum of 2 to 3 years of hands-on experience working directly with individuals, preferably children, diagnosed with complex medical needs.
  • Possession of a valid PA driver’s license and insurance for personal vehicle transportation.
  • Compliance with clearances mandated by the Pennsylvania Child Protective Services Law.
  • Demonstrated experience in project development and coordination, with a focus on collaborating with multiple human service entities and families.
  • Proven track record in delivering education to both adults and children, with the ability to tailor educational programs to meet the unique needs of participants.
  • Experience in teaching adults and children, coupled with a sense of urgency and flexibility in addressing the educational needs of families and children.
  • Desired qualification includes bilingual language skills.

Compensation

The compensation for this role is $62,900 annual salary. Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
