Home Health Physical Therapist

Join Expert Healthcare as a Home Health Physical Therapist, where you will have the unique opportunity to provide essential rehabilitative care to patients in their own homes. In this role, you will assess each patient's individual needs and establish personalized therapy plans aimed at enhancing mobility, strength, and overall well-being. As a part of our team, you will collaborate with healthcare professionals to deliver coordinated care and document patient progress meticulously. We value a nurturing environment where both our clients and staff can thrive. Monday - Friday. Coverage area is Hickman County and surrounding areas (roughly 7 counties but will be scheduled accordingly so visits are not widely spread each day)

Key Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans.
• Deliver effective therapeutic exercises tailored to patient needs.
• Track and record patient progress, adjusting treatments as necessary.
• Educate patients and families on rehabilitation practices and equipment.
• Collaborate with multidisciplinary teams to enhance patient outcomes.
• Maintain accurate records in compliance with healthcare regulations.

Requirements

• Valid Physical Therapist license in the state of practice.
• Bachelor's or Doctorate in Physical Therapy from an accredited institution.
• Experience in Home Health setting required.
• HomeCare HomeBase charting experience required.
• Exceptional communication and interpersonal skills.
• Ability to work autonomously and manage a diverse caseload.
• BLS certification is required.
