Village Green Memory Care Community is seeking a dedicated and compassionate Health and Wellness Coach to join our team. As a leading memory care community, we are dedicated to providing exceptional care and support for individuals with Alzheimer's and other forms of dementia. The Health and Wellness Coach will play a vital role in promoting the well-being and overall health of our residents.
The Health and Wellness Coach will work closely with our residents, their families, and our interdisciplinary team to develop and implement personalized wellness plans. In this role, you will provide support, education, and guidance to our residents in the areas of nutrition, physical activity, mental stimulation, and social engagement.
With your expertise and passion for senior care, you will help our residents maintain their independence and enhance their overall quality of life. You will have the opportunity to build meaningful relationships with our residents and be a trusted resource for them and their families.
Responsibilities
- Develop and implement individualized wellness plans for residents based on their specific needs and goals
- Conduct health assessments to determine residents' current health status and identify areas for improvement
- Provide education and guidance to residents on nutrition, exercise, and activities that promote overall well-being
- Coordinate and lead group exercise classes and wellness programs
- Collaborate with the interdisciplinary team to ensure that residents' needs are met and their goals are being addressed
- Monitor residents' progress and make necessary adjustments to their wellness plans
- Provide emotional support and encouragement to residents and their families
Requirements
- Bachelor's degree in health science, nutrition, exercise physiology, or a related field
- At least 2 years of experience in a similar role, preferably in a senior care setting
- Strong knowledge of Alzheimer's and dementia care guidelines and best practices
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a team environment
- Proficient in Microsoft Office and electronic health records systems
- A commitment to providing person-centered care and promoting the well-being of older adults
