Job Summary
Facilitate the flow of communication between caregivers, patients, and doctors. Provide legendary customer service to ensure positive relationship with referral sources.
Essential Functions:
- Demonstrate current and up-to-date knowledge of all state and federal governmental regulations and intermediary guidelines.
- Demonstrates proficient understanding and adherence to continuous quality improvement objectives and safety, environmental or infection control standards.
- Establish relationship with physicians and staff to encourage patient referrals.
- Obtain referral information on hospitalized patients when contacted by case managers and establish initial contact with patient and family.
- Follow active patients when hospitalized or in a long-term facility on temporary basis.
- Follow developed focus list.
- Log Therapy visits and submit referrals when received.
- Answer patient related phone calls as needed and handle accordingly.
- Assist office in obtaining physician orders as needed.
- Serve as liaison between staff and referral sources.
- Travel to referral sources to perform job tasks.
- Coordinate agency involvement in local health fairs and other programs in order to keep the community aware of agency’s presence and purpose.
- Maintains compliance with all New Age's policies, procedures, and regulations, including all federal and state laws.
- Maintains a clean and safe work environment.
- Accurately prepares and proofreads letters, memos and other correspondence.
- Review and accurate processing of mail and other correspondence daily.
- Performs all other duties as requested by supervisor.
Requirements
- High School Diploma or Equivalent
- Requires minimum of one year prior successful experience as a Community Coordinator or comparable position. Home nursing care experience preferred.
- Requires ability to speak, read and write English
- Requires ability to communicate clearly and make self understood effectively in face to face interactions; articulate with accuracy to communicate using phone.
- Requires ability to effectively interact with and coordinate service delivery both internally with staff and externally with the community at large including physicians, patients, family members, vendors, insurers, other providers, etc.
- Must have and maintain current valid Texas Driver’s license and proof of current auto liability insurance as this position requires travel to various geographic locations
