SUMMARY:
The Case Manager will work on assignment through our partnership with the Home & Community Service (H&CS) program of the Chester County Intermediate Unit (CCIU). The Case Manager will assist eligible individuals in gaining access to necessary medical, educational, rehabilitative, and social services; assist in monitoring service delivery. The Case Manager will be responsible for: all clerical duties related to the position, participate in the client intake process and the orientation of clinical staff; ensuring the timely development and submission of client-centered plans; promoting positive relations with referral, regulatory and funding agencies, and staffing partners; and promoting the integration of children with disabilities in the community.
ASSIGNMENT:
- Schedule: M-F, 7:30am - 3:30pm
- Duration: 12 weeks
- Compensation: $31 per hour
Requirements
EDUCATION / EXPERIENCE:
- High School Diploma or higher
- Insurance and case management / case specialist related experience
CREDENTIALS:
(Clearances current within 12 months)
- PA Child Abuse (Act 151) Clearance
- PA Criminal (Act 34) Clearance
- PA Department of Human Services FBI – Use code 1KG738
- TB Screening (current within 3 months)
- Education Documentation - Diploma OR Official Transcripts
