The Road Home has been a leader in the fight to end homelessness for 100 years (1923-2023). We provide low-barrier emergency shelters, supportive services, and housing-first-minded rental assistance that helps individuals and families step out of homelessness and back into the community. Job Summary
The Case Manager will work as part of our Palmer Court case management team to provide direct case management and supportive services to individuals at the Wendell Apartments and individuals and families at Palmer Court who have moved out of homelessness and are living in permanent supportive housing at the Wendell and Palmer Court.

Duties and Responsibilities
- Assist clients in developing goals that will help them overcome homelessness and stabilize in permanent supportive housing with the opportunity to move on to independent housing.
- Provide residents with referrals and link them to community resources.
- Provide best practice case management to singles and families, including crisis intervention, ongoing assessments, goal setting, problem solving, advocating, agency and community resource utilization, and network building.
- Work professionally and cooperatively with case management team, property management, partners, and community resource providers.
- Attend weekly staff/staffing meetings and other meetings as assigned.
- Keep records, case management notes, and statistical data as required and participate in research and/or special projects as needed.
- Assess clients’ barriers to self-sufficiency and develop problem-solving skills with the client.
- Evaluate one’s own performance to increase effectiveness as a case manager.
- Other duties as assigned.

Requirements
Education and Experience
- Experience working with diverse populations preferred.
- Must have either lived/worked/volunteer experience or a degree in a related field.
Required Skills and Abilities
- Strong interpersonal skills and ability to work with diverse populations.
- Excellent writing and documentation skills.
- Willing to be a proactive member of a team.
- Ability to work from a Trauma-Informed Care framework.
- Familiarity with the Housing First philosophy.
- Ability to work with community partners.
- Self-starter and self-directed.
- Ability to accept direction from Supervisor.
- Excellent organizational skills and strong computer skills.
- Demonstrated awareness of and sensitivity to diverse populations. Ability to contribute to the agency’s commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages.
- Must pass a pre-employment background check and drug screen.
Physical and Equipment Requirements
- Ability to lift 25 pounds. Ability to stand and walk for at least an hour at a time.
- Ability to drive a vehicle. Must be licensed and insurable by our agency insurance carrier.
