Job Title: SMI Primary Care Advanced Practitioner
Overview:
Our company is looking for a compassionate and experienced SMI Primary Care Advanced Practitioner to join our team. We are dedicated to providing whole-person care to individuals living with Serious Mental Illness (SMI) who have historically been underserved, stigmatized, and deprioritized. Our goal is to build a trusting relationship with our patients and support them in accessing quality care. The SMI Primary Care Advanced Practitioner will be responsible for conducting whole-person clinical assessments, leading the medical component of team-based care, and supporting care coordination among medical and behavioral health providers.
Responsibilities:
• Conduct whole-person clinical assessments, including gathering information from outside sources, assessing both behavioral and physical health conditions through a thorough history and physical examination, capturing conditions not yet diagnosed, organizing information to support successful disability applications (if appropriate), conducting visits in the field or in an individual’s home when needed, and making individualized recommendations and/or referrals for programs/care provided by outside providers based on assessed need.
• Lead the medical component of team-based care by training and advising Guides and Community Resources Guides on clinical issues and treatments, supporting the Guide Team on care coordination, identifying actionable care gaps, performing medication reconciliation post-discharge and as needed, and communicating and coordinating with individuals’ care providers primarily at CMHCs, FQHCs, and hospitals.
• Collaborate with the interdisciplinary team to develop and implement individualized treatment plans for patients with SMI.
• Provide clinical support to Guides (peers) and Community Resources Guides (social workers).
• Maintain accurate and timely documentation of patient care in the electronic medical record system.
• Participate in quality improvement initiatives and ensure compliance with all regulatory requirements.
• Attend regular staff meetings and participate in ongoing education and training opportunities.
Requirements
Requirements:
• Master’s degree in Nursing or Physician Assistant with a specialization in Mental Health or Family Practice.
• Unrestricted license to practice as a Nurse Practitioner or Physician Assistant in the State of which this job is posted.
• Minimum of 5 years of clinical care experience as a Nurse Practitioner or Physician Assistant, ideally in a primary care setting.
• Experience working with and managing underserved populations with challenges such as behavioral health and/or SUD conditions.
• Strong knowledge of trauma-informed care, social determinants of health, and cultural humility.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work well in a team-based, interdisciplinary environment.
• Proficiency with electronic medical record systems and basic computer skills.
• Valid driver’s license
