Seeking a Registered Nurse to become our Home Health RN Case Manager in the Manhattan, KS and surrounding areas!

As a Phoenix RN Case Manager, you will be the first point of contact between a patient and hospital. Our Home Health RNs should possess strong skills in physical assessment, diagnostic interpretation, and communication to formulate an individualized plan of care according to physician orders that incorporates the analysis of initial assessment. Working alongside other nurses, Physical Therapist, Speech Therapist and or Occupational Therapist.

Pay Range: $65,000 - $90,000 based on 30+ units.

Purpose: As a Home Health RN Case Manager at Seasoned Recruitment, you will be responsible for administering skilled nursing care to patients on an intermittent basis in their place of residence.
